As we have recently announced, we will be temporarily implementing two additional safety measures for our in-person worship gatherings.  

• Firstly, the form linked above will act as a health screening form and as a means of registering to attend in-person worship gatherings.  We will be implementing a maximum capacity of 50 people at the Symphony Church Office and therefore this form will allow us to best prepare and plan for your visit. Please fill out this form prior to arriving for Sunday Service. 
• The second safety measure we will be implementing is regarding travel policies that will be put into place for the remainder of January. Additional details are outlined in the 'Travel' section in the form.
